Thank you for choosing Premier Eye Care for your vision care needs. Due to the COVID-19 pandemic, we have made several adjustments to our office procedures. These protocols are in place in order to protect the health and well-being of our staff and patients, to ensure optimal efficiency in our schedule, to streamline our appointment process, and to abide by federal, local, and CDC guidelines. Please read and reply “YES” by text to acknowledge that you understand and agree to our updated procedures. If you have any questions or concerns, please feel free to call our office.
New patients must complete our registration packet online, OR arrive 20 minutes early to fill it out prior to the appointment time. If the paperwork is not completed prior to the time of your scheduled appointment, you may be asked to reschedule.
Please confirm your appointment. You will receive a text message asking you to confirm. If you do not confirm via text, we will call you to verbally confirm. If we are unable to confirm your appointment no later than 24 hours ahead of time, your appointment will be cancelled. This policy is to ensure that we do not have missed open appointment slots, as we currently have a waiting list for appointments.
If for any reason you need to reschedule or cancel your appointment, please let us know as soon as possible. If you do not show up to your appointment, you will be charged a $25 fee. If you want to reschedule the missed appointment, that fee will be due at the time of scheduling. Any appointments for 2 or more people that are missed will be required to schedule on separate days in the future. We do understand that emergencies arise, and that you may be unable to notify us ahead of time that you will not make it. We will address that on a case-by-case basis.
PLEASE ARRIVE 10 MINUTES PRIOR TO YOUR APPOINTMENT TIME. Please allow yourself extra time to ensure you are on time to your scheduled appointment. Due to COVID precautions and social distancing protocols, anyone arriving late to their appointment WILL be asked to reschedule.
In order to ensure proper social distancing protocol, the door to our office will remain locked at all times, and entry into the office will be only be permitted by appointment only. This includes glasses and contact lens pick-ups.
When you arrive for your appointment, please remain in your vehicle and call or text our office to let us know you have arrived. We will notify you when we are ready for you to come inside.
At this time, we are allowing only the patient scheduled for the appointment to come into the office. If the patient is a minor child, one parent may come in with them. If the patient needs medical assistance, then one other person may come in with them as well.
Masks or face coverings are required, they must cover your mouth AND nose, and must be worn at all times while in the office. This includes while trying on glasses. If you are unable to wear a mask, we will schedule you when face masks are no longer required.
We understand that these procedures may cause a slight inconvenience to our patients. We thank you for your cooperation and understanding during this unprecedented time. The health and safety of our staff and our patients remains our top priority. These new protocols will help ensure that we are providing the safest environment possible, while upholding the excellent standard of care that we pride ourselves on.
