These protocols are in place in order to protect the health and well-being of our staff and patients, to ensure optimal efficiency in our schedule, to streamline our appointment process. Please read and reply “YES” by text to acknowledge that you understand and agree to our updated procedures. If you have any questions or concerns, please feel free to call our office.
New patients must complete our registration packet online, OR arrive 20 minutes early to fill it out prior to the appointment time. If the paperwork is not completed prior to the time of your scheduled appointment, you may be asked to reschedule.
Please confirm your appointment. You will receive a text message asking you to confirm. If you do not confirm via text, we will call you to verbally confirm. If we are unable to confirm your appointment no later than 24 hours ahead of time, your appointment will be cancelled. This policy is to ensure that we do not have missed open appointment slots, as we currently have a waiting list for appointments.
If for any reason you need to reschedule or cancel your appointment, please let us know as soon as possible. If you do not show up to your appointment, you will be charged a $25 fee. If you want to reschedule the missed appointment, that fee will be due at the time of scheduling. Any appointments for 2 or more people that are missed will be required to schedule on separate days in the future. We do understand that emergencies arise, and that you may be unable to notify us ahead of time that you will not make it. We will address that on a case-by-case basis.
